Since 2005 when Community Council of Idaho opened the doors of its first Community Family Clinic, the mission has been to:

Provide quality affordable primary healthcare services to individuals and families in the communities we serve.

We strive to fulfill this mission regardless of socioeconomic status, health insurance status, financial situation, race, religion, national origin, age, gender or sexual orientation.  Our team of dedicated healthcare professionals work everyday to provide our patients with quality affordable preventive healthcare.

The clinic accepts uninsured patients, patients with private insurance, and patients with Medicaid or Medicare. Community Family Clinic is an “assigned Medicare provider”.  For low-income patients, services may be provided on a sliding fee scale to those that qualify.  Qualification is based on income, so please bring proof of income to your appointment.

All services are available in Spanish and English.



Family Medicine Services

Community Family Clinic provides full family medicine services.  Our services include:

  • Annual exams for men, women, and children
  • Minor office procedures
  • Preventative health services
  • Behavioral Health Services
  • Chronic disease management; including diabetes and hypertension
  • Immunizations for children and adults
  • Discounted lab services
  • Access to discounted medication
  • Birth control
  • Allergy treatment
  • Health Insurance enrollment assistance

*Expanded Medicaid is available for qualifying low-income adults*

Partnering for your Health

Community Family Clinic has partnered with the Eastern Idaho Regional Medical Center’s (EIRMC) residency program! EIRMC residents are now providing medical services at Community Family Clinic. The resident team practices under the direct supervision of Dr. Leland Krantz MD (Internal Medicine,) Dr. Alan Avondet MD (Internal Medicine and Nephrology (kidney disease,) Dr. Jaren Blake MD (family medicine,) and Dr. Brian Jones DO, MPH (family medicine).  Currently the residents see patients on Mondays, Tuesdays, Wednesdays, and Fridays at our Idaho Falls CFC.

Our partnership with EIRMC has expanded the clinic’s capacity to serve more patients. More appointments are available, usually within a week, and more walk-in slots are open. The entire SE Idaho community benefits from our partnership with EIRMC residency program. Thank you, EIRMC!

Community Family Clinic’s Medical Director(s) maintain admitting privileges to EIRMC and work with the hospital’s residency program in service delivery across all SE Idaho locations. EIRMC also refers unnecessary emergency room visits to CFC. 

We provide Shriners Hospital for Children (SHC) Telehealth Network in our clinic, so patients who live in SE Idaho can connect through video consultations with their Salt Lake City, Utah physicians without the cost and time of travel.

  • Community Family Clinic partners with the Prescription Center to offer discounted prescriptions to our patients.
  • Quest Diagnostics and Treasure Valley Labs offer significant discounts on all lab work for our patients
  • Mountain View Hospital provides discounted rates on all imaging and x-rays to patients
