Sliding Scale Fee

CFC offers vital health services to patients based on a sliding-fee scale. Discounts are available for community members who qualify, taking into account their income and household size. You can fill out an application at any of our offices or print the application using the link below:

To apply, please come to your appointment prepared with:

  • Photo ID
  • Proof of Address
  • Income verification for all working members of the household.

Valid proof of income includes:

  • Income Tax Returns for previous year.
  • Paystubs for the last 30 days.
  • Bank Statement for the last month.
  • Work verification letter or Termination of Employment letter.

Insurance Enrollment Counseling

Our enrollment counselors provide guidance on the Medicaid program,
including eligibility and application assistance for you and your family.
They also offer information about affordable health insurance options through Your Health Idaho,
including potential tax credits to help with premiums.
